Appointment Policies & Courtesy Reminder
We understand that life happens, but to ensure the best care for all our patients, we kindly ask for at least 24 hours' notice if you need to cancel or reschedule your appointment. This allows us to accommodate other patients who may be waiting for an opening.

Patient Appointment Checklist
- Arrive Early. Please arrive 15 minutes before your scheduled appointment and call us from the parking lot upon arrival.
- Complete Patient Questionnaire. Complete our patient questionnaire in advance and bring it with you.
Forms are available for download on this page. - Attend Alone When Possible. To ensure efficiency, please attend your appointment alone unless you require physical assistance or are accompanying a minor.
- Bring Your Glasses or Contacts. Bring all glasses you currently wear or the boxes of contact lenses you use.
Ensure you are wearing the same contact lens prescription you provided us. - Provide a List of Medications. Bring a complete list of all medications you take, including vitamins and supplements.
- Bring the forms with you on the day of your exam.
